It is a modern retro-interpretation of the classic theme "Horror and Nightmare of the overstressed and near burnt out Head-Shrink" where the fears and nightmares are shown that an -in this case female- psychiater can have if he or she overworks and has no release-valve for him or her self. It clearly rakes back to the Jungian and Freudian psycho-analythic approaches of looking at the expert as him or her only helping others to handle his or her own personal problems with his or her own parents and house-pets.
I like the accents the artist has used like the little white knob on the fan, the bithmark on the temple of the fear-struck docter and the silvery bits on the belt of the Magay.
So, I think that thing on her monitor is a Santa Claus hat, so maybe it's Christmastime? But there's a fan on her desk, so it's hot weather; I guess they're Australians. If it's Christmas, perhaps his costume is related to a pageant (note that his turban really is a towel; you can see the tag on it. And I think the robe is a bathrobe). I guess he's playing the part of a shepherd... or perhaps Joseph. Could it be that the sphinx cat is playing the part of the baby Jesus?!

he is doing a show only as excuse to show his nipple-piercings.
There's a lot going on in there. What's on her desk? And draped over her computer monitor?
